Demonophobia is a disturbing guro hentai game with some element of lollicon, involving death, torture and dismemberment with very crude scenes. As such it is recommended for ages 21 or up. The game's protagonist is the girl Sakuri who tries summoning demons for a personal vengeance. She imagined it as a game but the ritual succeeds and she ends up in hell. Although there is direct control over her actions, the player is cast as the audience who witnesses the story. She had to go to six levels do undo the ritual and save her village and people.
The game is of the 2D side-scrolling kind with different screens linked together in a maze-like fashion. She has very little means of defence and instead needs to avoid enemies by running past them, crouching or quickly moving to a new area. The author appears to aim for shock value, as the game features numerous gruesome long death sequences with a disturbing amount of detail. As Sakuri is in hell, she never really dies but instead needs to replay the same sequences. At certain moments a checkpoint is provided. The girl has a limited amount of health. As she is hit by enemies, the border colour of her portrait and the facial expression on it is altered.
All six levels offer an end boss that requires quick reflexes to kill, with the final one being Lucifer, presented as a deformed butcher. Sakuri can also pick up items to open doors, restore health and there are a few scarce weapons. Collected items are stored at the bottom of the screen and can be selected at any time. The game features explicit nudity with sadistic elements. At certain parts of the game, the girl is entirely naked as she battles the demons. There is no sound or music in the game.
